2012 - 2013 School Registration

2012 -2013 Registration is now open!

Registration for the 2012-2013 academic year is now open.  You may request a Kindergarten packet by calling the school office at 314-352-9474.  Forms are available below as well

Your packet includes the following forms to fill out and return to the school office:

• Admissions Form (New Elementary or Preschool students)
• Student Assessment Form (for students entering Kindergarten, to be filled out by their preschool)
• Request for Information From Current School (for transferring students)
• Financial Letter of Intent (plus $200 per child book/technology fee, K-8 only)
• Eagle Bank Tuition Payment Agreement (plus voided check)
• Club Tobias After Care Enrollment Form (optional)
• Summer Camp 2012 Enrollment Form (optional)
• Snow Day Options 2012 Registration Form (optional)

Please also include the following when turning in your registration:

• Include Baptismal Certificate if your child was baptized at a Church other than St. Raphael (K-8)
• Include a copy of your child’s Birth Certificate (K-8)
• For non-Catholics applicants, please obtain a letter of recommendation from your minister.  If, at present, you do not have a religious affiliation, please indicate that in a note to us.

Financial Aid Forms are available at the rectory upon request.  Please call 314-352-8100 for more information.

For grades K-8, to avoid a $20 registration fee, please turn in your Financial Letter of Intent by February 28, 2012.  There is a $200 book fee/technology fee per student. This fee must be paid by May 1, 2012.

Please note: The Preschool Registration form has a $75 registration fee.  There is not a book fee for PK students. Financial Letters of Intent do not need to be completed for PK only.
